If you're pushing into the endgame of Path of Exile 2 and working your way through The Grand Expedition questline, you will eventually find your path blocked by Vorana, Last to Fall. Tied to the Black Scythe Mercenaries faction, Vorana is a mechanical hurdle that completely changes how you think about positioning. Unlike open-arena encounters where you can just run away from danger, Vorana traps you in an incredibly tight space and tests your mechanical execution. Defeating her is an essential step for late-game progression, as she drops Uthred's Logbook, which is required to advance the questline.
How to Unlock and Find Vorana
You won't stumble upon Vorana during the standard campaign or while casually clearing regular maps. Reaching her underground lair requires a specific set of steps:
Defeat Medved First: Your journey begins by progressing through the initial steps given by Dannig and Makoru in the Ruins of Kingsmarch. This gets you Makoru's Logbook. Follow this path to locate and defeat Medved, the Fallen Seer. Upon his death, he will drop Vorana's Logbook.
Expose the Islands: Take Vorana's Logbook back to Kingsmarch and activate it on your Atlas. This action reveals the hidden Black Scythe Islands on your map interface.
Roll the Map: Look closely through the newly revealed ocean island nodes for one specifically labeled "The Last to Fall..." (this typically features a Mournful Cliffside layout). Keep in mind that you will need a Tier 11 or higher Waystone to actually open and enter this map.
Detonate the Tomb: Once inside the zone, track down Tujen. When placing your explosive detonation chain, plan your path carefully. You need to ensure that your final explosives blow open Vorana's Tomb, which uncovers her underground lair.
Quick Tip: Double-check your build's single-target damage and physical mitigation before clicking that final entrance. The arena is incredibly unforgiving if the fight drags on too long.
Core Boss Mechanics & Strategies
The defining element of theVorana fight is severe spatial restriction. You have almost no room to breathe, meaning passive running around will quickly get you killed. Here is exactly what she does and how to handle it:
The Black Reaping (Dark Fog): The moment Vorana spawns, she summons a swirling dark fog that immediately closes in on her location. This forces you to stay inside a very small safe circle right around her. Do not step out into this fog; doing so inflicts a rapidly stacking damage-over-time debuff that can easily kill your character within one or two seconds. Treat the edge of the circle as a hard wall.
Lacerate & Double Scythe Slash: Vorana slashes her twin scythes in a massive semi-circle cross pattern directly in front of her. She gives a distinct audio cue, shouting "I shall cut you down!" right before unleashing this attack. Because the dark fog keeps you at close quarters, the best response is to constantly circle behind her back whenever she winds up.
Cyclone: When she shouts "Death Wind!", she lunges forward in a straight line while spinning continuously. Do not try to block or tank this move. Instead, quickly step laterally out of her direct path of travel and let her spin past you.
Orbiting Stars: Star-like orbs will rotate around the perimeter of your small safe zone, firing projectiles at you. If your character runs directly through an orb, it drops to the ground and detonates after a short delay. You can trigger these intentionally to clear them out, but only do so when you have enough space to safely step away from the ensuing explosion.
Ambush: Periodically, Vorana will vanish completely into the mist and become immune to all damage. While she is gone, keep your eyes on your minimap—her icon remains visible. You can also look for faint wind indicators blowing toward the center of the arena to predict exactly where she is about to re-emerge, giving you a split second to position yourself behind her.

Surviving this intense encounter pays off with some highly valuable endgame progression items:
Uthred's Logbook: This is the main prize. Take it back to Kingsmarch to unlock the next major leg of the questline, setting up your eventual confrontation against Uhtred, the Stardrinker.
